Key Responsibilities:

Collaborate with Social Workers, Occupational Therapists, and healthcare professionals to uphold individuals' independence and community ties.

Conduct assessments and develop support plans in accordance with the Care Act 2014.

Assist individuals in identifying and achieving their personal goals, promoting their independence.

Encourage multi-disciplinary collaboration within a team-oriented environment.

Candidate Profile:

Strong communication skills are essential, along with the ability to excel in a fast-paced environment and work effectively with a diverse range of professionals.

Experience in social care settings is preferred, although transferable skills are also valued.

Comfortable engaging with new individuals and adaptable across various social care environments.

Passionate about fostering independence and knowledgeable about key legislation and regulations.

Exhibit behaviors that reflect responsibility, respect, recognition, and collaboration.

Must be able to meet the travel requirements of the position.
