HILDEBRANDT HOSPICE CENTER

The Hildebrandt Hospice Care Center is an 11-bed facility dedicated to providing compassionate end-of-life care.

Nurses, Evaluators, and Hospice Aides collaborate closely with patients and their families during challenging times.

We work alongside hospitals and healthcare facilities to ensure seamless patient care.

QUALIFICATIONS
Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
Empathetic, warm, and patient-centered approach
Strong critical thinking abilities, decisive judgment, and capacity to work independently
Outstanding documentation skills and professional demeanor
Ability to thrive in a fast-paced environment

KEY RESPONSIBILITIES
Patient Care & Service. Enhance and restore patients' health by executing the nursing process; collaborating with physicians and multidisciplinary team members; performing various treatment procedures; providing physical, educational, and emotional support to patients, families, and friends; supervising assigned team members.
Planning & Communication. Formulate and document personalized care plans tailored to each patient's specific needs, with support from the interdisciplinary health team as necessary; maintain effective communication to relay patient health status, treatment plans, and progress.
Electronic Health Record (EHR) Management. Exhibit proficient use of an EHR, ensuring accurate patient and provider documentation and communication.
Compliance. Follow required department and system protocols, regulations (local, state, federal), and educational requirements.

PHYSICAL DEMANDS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing, or squatting.


CERTIFICATIONS/LICENSES:
RN - Registered Nurse - New York State Education Department (NYSED)
BLS - Basic Life Support - American Heart Association (AHA)

COMPENSATION RANGE:
$ $48.00
The stated base pay range reflects a good faith representation of potential base pay for successful candidates. It may be adjusted in the future. Compensation is influenced by factors such as experience, relevant qualifications, specialty, internal equity, location, and contractual agreements.
