YOUR ROLE (ESSENTIAL DUTIES)...

We are currently in search of a dedicated, compassionate, and innovative full-time Vocational Rehabilitation Consultant to join our Healthy Transitions Team, focusing on youth and young adults aged 16 to 26.

This role involves providing vocational rehabilitation services to designated clients, utilizing the Individual Placement and Support (IPS) model to facilitate competitive job placements, educational opportunities, and meaningful daily routines.

Key responsibilities include:

  • Collaborating with clients to identify and pursue employment and educational aspirations based on their interests, strengths, skills, and capabilities.
  • Assisting clients in recognizing and overcoming obstacles that hinder their ability to secure or maintain competitive employment or educational pursuits.
  • Providing ongoing support for career exploration, job development, and job retention.
  • Networking and communicating with employers to offer on-the-job support as needed.
  • Conducting vocational assessments, evaluations, and career profiling.
  • Encouraging client involvement in community activities to enhance social networks and community integration.
  • Working with clients on their goals as outlined in their recovery plans.
  • Engaging with families and identified support systems integral to the client's recovery journey.
  • Co-leading vocational groups alongside a Peer.
  • Facilitating the development and securing of new job opportunities through collaboration with the Office of Rehabilitation Services and community employers.
  • Providing job coaching within our transitional and supported employment programs.
  • Collaborating with treatment teams to deliver essential case management support.
WHAT WE EXPECT FROM YOU...
Minimum of six months of relevant experience required.
Knowledge of psychosocial rehabilitation, recovery treatment planning, vocational assessment, skills training, and job placement/development.
Exceptional communication abilities, both verbal and written, to engage with a diverse range of individuals professionally.
Empathetic and caring disposition, with a genuine desire to assist those in need.
Self-driven and independent, with the capability to prioritize tasks effectively to meet deadlines.
Valid Driver's License and reliable transportation.
Bachelor's degree in a human services field and SEP certification preferred.
Familiarity with the IPS Model and Evidence-Based Practices preferred.
