Physical Therapist Assistant

Middletown Park Rehabilitation & Healthcare is seeking a skilled Physical Therapist Assistant to join our team. As a Physical Therapist Assistant, you will play a vital role in the rehabilitation of our residents, working closely with our Physical Therapist to develop and implement effective treatment plans.
Key Responsibilities- Organize and conduct physical therapy programs to facilitate rehabilitation of residents with decreased functions.
- Assist in planning, organizing, and implementing physical therapy services under the supervision of the Physical Therapist.
- Perform treatment programs for assigned residents, ensuring the highest degree of quality resident care.
- Enter billing information accurately and in a timely manner.
- Report resident progress in meetings, as required or appropriate.
- Consult with other rehabilitation team members to coordinate physical therapy with other therapeutic activities.
- Provide residents with equipment as necessary.
- Chart notes in an informative and descriptive manner, reflecting the therapy provided and the resident's response.
- Perform routine documentation, signing and dating all entries in residents' medical records.
- Cooperate with all departments and staff to promote harmonious working conditions and optimal resident care.
- Graduate of an accredited school of Physical Therapy Assistant.
- Licensed in NY State as a Physical Therapist Assistant, with a license in good standing.
- Previous skilled nursing facility experience is highly preferred.
- Ability to work well under pressure and harmoniously with all staff.
- Sincere desire to work with the elderly, chronically ill, and handicapped.
- Excellent customer service skills.
- Ability to communicate effectively with residents, families, and staff.
- Treat residents and fellow staff with respect, dignity, and compassion.
- Covid-19 vaccinations are encouraged but not mandated.
