Job Title: Quality Improvement Manager

We are seeking a highly skilled Quality Improvement Manager to join our team at Mountain View PACE. As a key member of our quality improvement team, you will play a critical role in developing and implementing quality improvement initiatives that drive excellence in patient care.

Job Summary:

The Quality Improvement Manager will be responsible for leading the development and implementation of quality improvement initiatives, including data analysis, quality indicator development, and education and training of staff. This role will also involve coordinating with national initiatives in PACE quality management and ensuring compliance with regulatory standards.

Key Responsibilities:
  • Develop and implement quality improvement initiatives to drive excellence in patient care
  • Lead data analysis and quality indicator development to inform quality improvement efforts
  • Provide education and training to staff on quality improvement activities and techniques
  • Coordinate with national initiatives in PACE quality management
  • Ensure compliance with regulatory standards
  • Conduct audits of nursing documentation to ensure accuracy and compliance
  • Review participant records for completeness, consistency, and appropriateness of care
  • Analyze trends and patterns in documentation to identify areas for improvement
  • Develop and monitor systems to achieve quality measures
  • Promote quality result behavior in staff through observation, feedback, and education
  • Collaborate with leadership and interdisciplinary teams to develop and implement strategies for enhancing quality of care
Requirements:
  • Current RN License
  • Post-secondary education related to Nursing, Business, Social Services, or Healthcare
  • One year of experience in healthcare quality improvement/management, one year of experience working with the frail and elderly population
Physical Requirements:
  • Must be able to move intermittently throughout the workday
  • Must function independently, maintain confidentiality, have flexibility, personal integrity, and the ability to work effectively with personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and sometimes hostile people within the facility
  • Is subject to lifting, carrying, and supporting participants
