Hospice Patient Care Coordinator Job Description

Job Summary: We are seeking a skilled and compassionate Hospice Patient Care Coordinator to join our team at Interim HealthCare. As a key member of our hospice care team, you will play a vital role in ensuring the highest quality of care for our patients and their families.

Key Responsibilities:

  • Collaborate with the Director of Clinical Services to develop and implement processes that ensure efficient and effective patient outcomes.
  • Provide ongoing education to hospice employees on regulations, guidelines, and best practices in hospice and palliative care.
  • Supervise, evaluate, and motivate professional and paraprofessional employees to ensure exceptional patient care.
  • Use clinical decision-making to provide oversight and guidance to the healthcare team, ensuring seamless patient care.
  • Serve as an expert for the hospice admission process and perform admissions as assigned.

Requirements:

  • Graduate of an accredited school of nursing and licensed to practice as an RN in the state in which employed.
  • Two years of experience as a Hospice RN required, with at least one year of supervisory experience preferred.
  • Able to provide proof of valid driver's license and auto liability insurance in accordance with policy.
