About the Role:
As a Hospice Patient Care Coordinator with Interim HealthCare, you will play a vital role in providing oversight of the care and services furnished to our Hospice patients. This is a challenging and rewarding position that requires strong clinical and leadership skills.
Key Responsibilities:
- Collaborate with the Director of Clinical Services to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes in accordance with physician orders.
- Provide ongoing education to hospice employees on regulations, guidelines, current standards of hospice and palliative care practice, Interim HealthCare Hospice policies and procedures, quality assessment and performance improvement (QAPI) programs, and each employee's role in each.
- Supervise, evaluate, motivate, and develop professional and paraprofessional employees.
- Use clinical decision making to provide oversight and guidance to the healthcare team to efficiently and effectively manage the assigned patient population.
- Serve as an expert for the hospice admission process and perform admissions as assigned.
Requirements:
- Graduate of an accredited school of nursing and licensed to practice as an RN in the state in which employed. License is active and in good standing.
- Two (2) years of experience as a Hospice RN required and at least one year of supervisory experience preferred.
- Able to provide proof of valid driver's license and auto liability insurance in accordance with policy.
