We are seeking a highly skilled Coordinated Care Coordinator to join our team at MedElite. As a Coordinated Care Coordinator, you will be responsible for coordinating patient care, collaborating with healthcare providers, and ensuring seamless care transitions.

Responsibilities:

  • Coordinate patient care, develop and implement personalized care plans.
  • Collaborate with healthcare providers to ensure seamless care transitions, resolve issues, and address concerns in a timely and professional manner.
  • Conduct regular assessments to ensure alignment with industry best practices and identify areas for improvement.
  • Develop and implement quality assurance measures to guarantee delivery of high-quality services.

Requirements:

  • 2+ years of experience in care coordination or a related field, strong communication and interpersonal skills.
  • Proficiency in Microsoft Office applications, ability to work in a fast-paced environment.
  • Strong analytical and problem-solving skills, ability to prioritize tasks and manage multiple projects.
  • Experience working with Electronic Medical Records (EMR) systems preferred, familiarity with regulatory requirements.
