Care Coordinator

Senior TLC is seeking a skilled Care Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for assessing the home care needs of our frail elderly population and developing specific plans of care.
Key Responsibilities:
- Assess the home care needs of our frail elderly population and identify specific plans of care.
- Coordinate 24-hour care delivery and implement all home care services, including personal care services.
- Reconcile invoices for personal care service hours and home supplies.
- Coordinate and authorize all Durable Medical Equipment (DME) and manage home supplies.
- Support the Senior TLC mission to encourage and support the quality of life of seniors wishing to continue living in the community.
Requirements:
- Bachelor's degree or equivalent experience.
- Minimum of 1 year's experience working with the frail elderly population preferred.
- Current, valid NC driver's license and vehicle.
- Excellent communication, interpersonal, and conflict resolution skills.
