At The Springs at Wyandot Trail, we're seeking a skilled MDS Coordinator to join our team. As a key member of our care team, you'll play a vital role in assessing residents' needs and creating personalized care plans.
Key Responsibilities:
- Manage the Resident Assessment Instrument (RAI) process to ensure accurate and comprehensive assessments
- Promote cross-functional communication to ensure care plans are followed and residents receive high-quality care
- Ensure appropriate reimbursement for resident care services
- Stay up-to-date on current federal, state, and local standards, guidelines, and regulations
Requirements:
- Current, valid state RN license
- 2+ years of MDS (resident assessment) experience
- Prior experience in Long-Term Care is required
