Key Responsibilities
  • Process client applications for financial assistance from multiple programs and services.
  • Provide exceptional customer service, navigating clients to additional resources and creating referrals for services in the department's CMS.
  • Review program applications for eligibility, ensuring clients receive the support they need.
  • Maintain all required documents in the program's electronic management information system.
  • Engage clients in Evidence-Based coaching interviews, utilizing related tools to identify the most appropriate level of services.
  • Provide information on community resources, establish information and referral mechanisms, and assist with program enrollment.
  • Attend community outreach and education events, promoting the services offered by Maricopa County, AZ.

Requirements
  • Two years of case management experience.
  • Certificate, credential, or associate degree in social work or a related field.
  • An equivalent combination of post-secondary education in Social Work or a related field and job-related experience may substitute for the required education year after year.
