We are currently seeking an experienced Home Care Physical Therapy Assistant (PTA) to join our team. This per diem position offers a unique opportunity for a skilled professional to make a meaningful difference in patients' lives.

The successful candidate will provide patient care services along with direct treatment and follow-up as assigned by the Physical Therapist. Key responsibilities include:

  • Collaborating with the Physical Therapist to update patient care plans and ensure seamless coordination with physicians and families.
  • Developing and implementing patient re-evaluation schedules to track progress and adjust treatment plans accordingly.
  • Accurately documenting patient care data to reflect evidence of improvement, progress, or decline in patients' conditions.

To excel in this role, you'll need:

  • A completion of an accredited physical therapy assistant program and current State licensure.
  • A minimum of one year's therapeutic experience in a non-home healthcare environment; two years of experience is preferred.
  • A strong knowledge of theory and concepts related to the practice of physical therapy, with the ability to treat patients with existing physician orders.
  • CPR certification and excellent communication and organization skills.
  • A commitment to ongoing education, with a minimum of 12 hours of continuing education annually.
  • An ability to adapt to travel assignments and working conditions as needed.

Salary: $45,000-$60,000 per annum, commensurate with experience. Benefits include a competitive salary, opportunities for professional growth, and a collaborative work environment.
