We are seeking an experienced Home Care Physical Therapy Assistant (PTA) to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care services along with direct treatment and follow-up as assigned by the Physical Therapist.

Job Description:

  • Providing Patient-Centered Care: Our PTA will work closely with physical therapists to develop and implement individualized care plans that cater to each patient's unique needs and goals.
  • Care Coordination and Communication: The ideal candidate will possess excellent communication skills, ensuring seamless coordination with healthcare professionals, patients, and families to ensure comprehensive care.
  • Data Documentation and Analysis: Accurate and timely documentation of patient care data is essential for effective care planning and outcome evaluation.
  • Scheduling and Visit Management: Our PTA will be responsible for submitting daily/weekly patient visit schedules to the scheduler, ensuring efficient management of patient visits.

Requirements and Qualifications:

  • Education and Licensure: Completion of an accredited physical therapy assistant program and current State licensure are mandatory.
  • Experience and Skills: A minimum of one year's therapeutic experience in a non-home health care environment, with a strong knowledge of theory and concepts relating to the practice of physical therapy.
  • Communication and Organization: Excellent communication and organizational skills are essential for success in this role.
