Residential Physical Therapist Position in Home Care Setting

Liberty Home Care offers a rewarding career opportunity for an experienced Physical Therapist to join our team as a Home Care Physical Therapist. This is a full-time position that requires a minimum of one year's therapeutic experience in a non-home health care environment.

We are seeking a dedicated professional who can provide patient care services along with direct treatment and follow-up. The ideal candidate will be able to evaluate patients new to service, develop a plan of care appropriate to the patient's diagnosis, and document patient care data accurately and thoroughly.

This role involves coordinating with Patient Care Coordinators, discussing company and third-party reimbursement policies and guidelines with patients and families, and providing in-service education, healthcare professional education, and public education as needed.

The successful candidate will possess a strong knowledge of theory and concepts relating to the practice of physical therapy, with the ability to evaluate and treat patients for whom a physician's order exists. A minimum of twelve hours of continuing education annually is required for this role.

Key Responsibilities:

  • Provide patient care services along with direct treatment and follow-up
  • Evaluate patients new to service and develop a plan of care
  • Document patient care data accurately and thoroughly
  • Coordinate with Patient Care Coordinators and discuss reimbursement policies with patients and families
  • Provide education and training as needed

Requirements:

  • Minimum bachelor's degree in physical therapy from an accredited school of physical therapy
  • Current licensure from the state where therapy will be provided
  • Prefer a minimum of one year's therapeutic experience in a non-home health care environment
  • Strong knowledge of theory and concepts relating to the practice of physical therapy
  • Ability to evaluate and treat patients for whom a physician's order exists
