Job Title: HIV Peer Navigator

Sea Mar Community Health Centers is seeking a highly motivated and compassionate HIV Peer Navigator to join our team. As a Peer Navigator, you will play a vital role in helping patients engage and remain in care for HIV treatment.

Key Responsibilities:

  • Provide one-on-one support and education to patients living with HIV.
  • Help patients overcome barriers to care, including social determinants and mental health challenges.
  • Collaborate with healthcare providers and community partners to ensure seamless care coordination.
  • Monitor patient progress and provide regular reports and data collection.
  • Develop and maintain effective relationships with a diverse patient population.

Requirements:

  • 1 year of experience in a related field, such as HIV/AIDS, healthcare, education, or outreach.
  • Ability to work with diverse populations, including ethnic, racial, economic, and sexually diverse individuals.
  • Strong communication and interpersonal skills.
  • Ability to maintain confidentiality and professionalism at all times.
  • Knowledge of patient care processes, clinical procedures, and medical terminology.

Preferred Qualifications:

  • Bilingual in English/Spanish or English/Russian.
  • Knowledge of the lived experience of living with HIV and willingness to share that experience.
