We are seeking a dedicated and compassionate Nurse Practitioner to join our team as a Gerontology Care Coordinator in Newington, Connecticut.

Job Overview

This 11-week local contract position offers an exciting opportunity to work with a renowned healthcare provider, delivering high-quality care to residents in need of gerontological services.

The ideal candidate will possess excellent clinical assessment skills, strong communication abilities, and a passion for patient-centered care.

Responsibilities
  • Conduct thorough clinical assessments for each resident on a monthly basis, with increased frequency for high-risk patients.
  • Collaborate with an RN to coordinate care, ensuring timely follow-ups, communication, and continuity of care across facilities.
  • Provide primary, preventive, and urgent care on-site, managing chronic and acute needs.
  • Facilitate conversations around advanced illness, setting personalized goals for each resident.
  • Identify and address gaps in care related to STARS, HEDIS, and COHPS measures, recommending adjustments to the primary care provider when necessary.
  • Prescribe or refill non-narcotic medications after consulting with the resident's primary care provider.
  • Offer single visits to patients transitioning from the hospital to the SNF, assessing and preparing them for a safe and successful discharge.

Estimated Salary: $120,000 - $150,000 per year based on industry standards and location.
