We are seeking a Primary Care Nurse to join our team at the Veterans Affairs, Veterans Health Administration. This is an exciting opportunity to work with a talented group of professionals in providing high-quality care to our nation's veterans.
About the RoleAs a Primary Care Nurse, you will be responsible for delivering patient-centered care to patients enrolled in our clinic. You will participate in patient education, preventive health interventions, management of acute and chronic disease states, telephone advice program, performance improvement activities, and case management.
Responsibilities- Provides initial and ongoing assessment of patients to identify needs, issues, care goals, and appropriate resources necessary for case management.
- Leads in the application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance/improvement.
- Functions as a systems coordinator for the plan of care; monitors progress through expected treatment courses and intervenes as appropriate to facilitate achieving patient outcomes within anticipated time frames.
- Accountable for utilizing critical thinking skills and sound judgment to ensure positive outcomes, maximize third-party collections, and deliver quality patient care performance improvement activities.
- Demonstrates knowledge of discharge planning and understands the level of care to match the veteran's/patient's needs with the most appropriate level of service.

RequirementsTo be considered for this role, you must have a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. A Master's degree in Nursing (MSN) is preferred.
