Care Coordinator for Individuals with Developmental Disabilities

The State of Connecticut is seeking a qualified Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing support and guidance to individuals with developmental disabilities. Your role will involve implementing programs and procedures designed to achieve the objectives of the department, as well as providing guidance and instruction to individuals in the development of desirable personal habits, hygiene, and social relationships.
Key Responsibilities
- Implement individual program plans
- Provide guidance and instruction to individuals
- Participate in the preparation, serving, and feeding of meals to individuals
- Perform housekeeping duties necessary to maintain a clean and healthy environment for individuals
Requirements
- One (1) year of experience in the care and supervision of individuals with developmental disabilities
- Possesses a Medication Administration Certification from the Department of Developmental Services
- Experience working with Adults with Autism and non-verbal individuals with intellectual disabilities
