Currently, we are seeking a compassionate LPN to provide ongoing care for a medically fragile child in an educational environment. Below are the specifics of this role:

Work Schedule: Monday, Wednesday, and Friday, 8:30 AM - 4:30 PM

Patient Age Group: Child

Key Responsibilities:
  • Implementing the physician's prescribed care plan
  • Conducting patient assessments and coordinating care
  • Administering prescribed medications, treatments, and therapies
  • Maintaining accurate and timely documentation of care provided
  • Promoting health and educating family members
  • Accepting and documenting verbal orders from physicians, ensuring co-signature from the Case Manager
  • Communicating with physicians, clients, family members, and staff regarding patient concerns and changes in condition
  • Performing additional duties as assigned
  • Assisting patients with transportation to and from school

Qualifications:
  • Active, unencumbered nursing license (RN/LPN/LVN) in the practicing state
  • Current CPR certification from recognized organizations
  • Recent TB skin test (within the last 12 months)
  • Preferred six months of hands-on nursing experience
  • Reliable transportation required
  • Fingerprinting may be necessary
