Advanced Practice Registered Nurse

We are seeking an experienced Nurse Practitioner to join our team at Centers Health Care. As a primary care provider, you will be responsible for assessing patient health, performing physical examinations, and obtaining medical histories. You will work collaboratively with our medical attending/physician and nursing home staff to provide high-quality care to our residents.
Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients' health, including physical examinations and medical histories, to develop individualized care plans.
- Collaboration and Communication: Work closely with the medical attending/physician, nursing home staff, and other healthcare professionals to ensure seamless care coordination and effective communication.
- Education and Support: Educate patients and their families on health promotion, disease prevention, and management of chronic conditions.
- Charting and Documentation: Maintain accurate and up-to-date records of patient care, including charting, documenting, and administering medications.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
- Education and Certification: Must be a Board Certified Advanced Practice Registered Nurse (APRN) with a Master's degree in Nursing.
- Experience: At least 2 years of experience in a long-term care setting, preferably as a Nurse Practitioner.
- Skills: Excellent clinical skills, strong communication and interpersonal skills, and ability to work effectively in a team environment.
