Position Summary

The Care Coordinator (LPN) is responsible for essential case management activities tailored to meet the needs of clients assigned to Care Lync Georgia. This role involves conducting initial assessments to determine the appropriate Level of Care, evaluating the suitability of Home and Community-Based Services (HCBS) for prospective clients, creating personalized care management plans, executing the care plan, reassessing client needs, and monitoring clinical outcomes to ensure that services are delivered in a professional, comprehensive, and cost-effective manner.

Key Responsibilities

  • Conduct thorough assessments to confirm or deny eligibility for nursing home level of care.
  • Perform medical and social evaluations under the guidance of a registered nurse.
  • Complete assessments in accordance with established guidelines and submit for review.
  • Possess experience in case management within the healthcare sector.
  • Maintain an active nursing license in the state of Georgia.

Assessment Responsibilities

  • Arrange face-to-face interviews with clients.
  • Gather medical documentation necessary for assessments.
  • Develop Comprehensive Care Plans tailored to individual client needs.

Submit completed assessments to the registered nurse supervisor for evaluation and recommendations.

Administrative Responsibilities

  • Assist in compiling statistical data.
  • Collaborate with the Management Team to schedule client appointments.
  • Participate in organizational meetings and training sessions.
  • Submit bi-weekly invoices for completed assessments.

Work Environment

Employees typically work in an office or residential setting with background noise.

Physical Demands

  • Ability to stand, walk, sit, reach, handle objects, climb, balance, stoop, kneel, crouch, or crawl as required.
  • Must possess effective reading, writing, and communication skills.
  • Regularly lift and carry up to 50 pounds.
  • Ability to push/pull up to 25 pounds as necessary.
