Job Title: Transition of Care RMA / LPN / RN

Cape Fear Family Medical Care is seeking a skilled Transition of Care RMA / LPN / RN to join their team. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions of care for our patients.

Job Summary:

  • Coordinate patient care transitions from hospital to home, ensuring continuity of care and minimizing readmissions.
  • Work closely with primary care providers, community agencies, and patients to develop and implement individualized care plans.
  • Provide education and support to patients and families on disease management, medication adherence, and prevention of complications.
  • Collaborate with healthcare teams to identify and address social determinants of health that impact patient outcomes.
  • Monitor patient progress, identify areas for improvement, and make recommendations for enhanced care coordination.

Responsibilities:

  • Develop and maintain relationships with community service providers to ensure access to necessary resources and services.
  • Assist patients in navigating the healthcare system, including scheduling appointments and arranging transportation.
  • Provide emotional support and advocacy for patients and families, addressing concerns and resolving issues.
  • Maintain accurate and timely documentation of patient interactions, care plans, and outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Degree in RMA or Nursing (RN or LPN)
  • Experience in direct patient care or managed care is highly preferred.
  • Valid driver's license and personal vehicle required for travel between work sites.
  • Excellent communication and customer service skills required.
  • Ability to work independently and collaboratively as part of a healthcare team.
