About the Role:

As a Patient Care Coordinator - Triage, you will be responsible for coordinating patient care, assessing patient needs, and providing guidance and support. You will work closely with other healthcare professionals to ensure seamless care delivery and high-quality patient outcomes.

Key Responsibilities:

  • Coordinate patient care and assess patient needs
  • Provide guidance and support to patients and families
  • Establish and maintain accurate medical records
  • Facilitate admissions and coordinate care delivery

Requirements:

  • Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) with current unrestricted Kansas license
  • 2-3 years home care or hospice experience preferred
  • Knowledge and skills in pain/symptom management, crisis intervention, and counseling
