Key Responsibilities- Conduct thorough psychosocial assessments to identify patients' needs and develop personalized plans for care.
- Collaborate with interdisciplinary teams to ensure seamless discharge planning and coordination of post-hospital services.
- Provide crisis intervention and short-term supportive counseling to individuals experiencing temporary or situational problems.
- Identify and address barriers to care, including financial, social, and emotional challenges.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care and support.
- Master's degree in Social Work (LMSW or LCSW) from an accredited program.
- Valid Louisiana State Board of Social Work Examiners licensure.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
