Hospice Social Worker

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Social Work PRN. As a Hospice Social Worker, you will play a vital role in providing emotional and practical support to patients and their families during the end-of-life journey.
Responsibilities:
- Contribute to patient and family counseling and education
- Participate in ongoing interdisciplinary comprehensive assessment
- Assess patient/family psychosocial response to the terminal illness and evaluate coping skills
- Assist patients, family, and agency staff in utilizing community resources
- Assist team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and family
- Coordinate with team members to plan and implement the hospice plan of care
- Participate in interdisciplinary team meetings
- Maintain accurate, thorough, and professional written records, and charts relevant information in a timely fashion, following UH documentation standards
- Participate in and is actively involved with the bereavement portion of the program as appropriate
- Participate in volunteer and staff education programs
- Develop and implement the discharge plan in conjunction with the IDS
- Participate in QAPI program and hospice-sponsored in-service training
- Refer high-risk patients
- Develop/maintain community resource file
- Incorporate organization's commitment to customer service and continuous quality improvement into daily work habits. This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agency
- Accept additional responsibilities as assigned
Requirements:
- MSW from a graduate school of social work is required. Must be licensed by the State of New York
- LMSW required, LCSW a plus
- Must have at least two years of supervised experience in the health field or two years of experience in family therapy/counseling
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Ability to relate to the terminally ill and their families; bereavement experience preferred
- Must be proficient in MS Office Suite, EMRs, and able to learn new software systems
- Must have daily access to a vehicle, a valid driver's license/acceptable driving record, and insurance coverage for daily travel to patient locations
Salary Range:
$31/hr. - 38/hr. (approx. $56,000-$79,000) and is commensurate with education, applicable experience, licensing, and certifications.
