About the Role

We are seeking a highly skilled Registered Nurse Clinical Assessment Advisor to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop personalized care plans to meet their unique requirements.
  • Collaborate with healthcare professionals, including physicians and other licensed clinicians, to ensure seamless care coordination and optimal patient outcomes.
  • Coordinate the delivery of care services, including home health aide support, to ensure patients receive the necessary support to maintain their independence and quality of life.
  • Monitor patient progress, identify areas for improvement, and revise care plans as needed to ensure the best possible outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure that all parties are informed and engaged in the care process.
Requirements
  • Current licensure as a Registered Nurse in the state of practice.
  • At least 2 years of experience in a clinical management role, preferably in home health care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Knowledge of state and federal regulations governing home health care, including PDGM.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
