About the Role

As a Clinical Assessment Advisor at CenterWell Home Health, you will play a vital role in ensuring the highest quality of care for our patients. Your primary responsibility will be to complete and oversee the development of treatment plans, as approved by physicians, and to perform ongoing evaluations of patient needs.

Key Responsibilities

  • Develop and implement comprehensive treatment plans that meet the unique needs of each patient.
  • Conduct ongoing evaluations of patient needs and coordinate with a team of professionals to ensure optimal clinical outcomes.
  • Work closely with physicians and referral sources to obtain all pertinent clinical information for assessment generation.
  • Ensure consistent team care scheduling and dissemination of updated clinical information.
  • Coordinate assigned care team members and resources to achieve established productivity standards and performance goals.

Requirements

  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of one year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Knowledge of clinical policies and procedures and ability to implement.
  • Knowledge of clinical structure of PDGM.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
