Job Summary

The Community Inclusion Specialist provides assistance to adults with developmental disabilities, creating and facilitating daily outings to promote community integration, build cultural identity, and connect participants with relevant opportunities to help meet their larger goals within the community.

Key Responsibilities
  • Escort, supervise, and oversee groups of up to 5 participants into the community on a daily basis to take part in activities and/or events.
  • Meet the basic needs of our participants while in the community, including medication administration and assistance with Activities of Daily Living (ADLs) as required.
  • Implement ISP's (Individual Support Plans) and behavior plans, as needed.
  • Create monthly budgets for outings and maintain tracking of receipts and expenditures for reconciliation.
  • Complete all required paperwork, including daily progress notes on a regular and timely basis.
  • Drive agency vehicles or use public transportation to assist individuals in accessing the community.

Requirements
  • High School Diploma, Bachelor's degree preferred
  • Three years' experience working with people with developmental disabilities
  • Basic Computer skills using Microsoft Products
  • Valid driver's license, regular access to a vehicle, and a clean driving record
