Key Responsibilities:
The Patient Care Coordinator - LPN facilitates effective communication among patients, their families, healthcare providers, and various medical facilities within the designated area.

Core Duties:
Utilizes the Electronic Medical Record (EMR) system to oversee, document, and manage all patient interactions.

Responsible for reporting non-urgent pathology findings for the assigned area, tracking pathology orders, and coordinating with laboratories to secure pending results. Responds to patient inquiries via phone and email, directing them to the appropriate healthcare professional as necessary.

Enters patient information and schedules appointments in the practice management system. Scans documents into patient records and forwards necessary paperwork to referring providers when required.

Assists in managing Prior Authorizations, prescription renewals, and returning patient communications while completing tasks within the Electronic Health Record (EHR). Aids clinical teams with various responsibilities such as preparing examination rooms, restocking supplies, rooming patients, scribing, and serving as a resource nurse as needed.

Engages with patients and their families to discuss treatment options, procedures, and ongoing care.
Provides information on community resources and referrals for continued care while ensuring confidentiality in managing patient accounts.
Maintains compliance with professional standards and regulatory requirements.
Collaborates effectively with team members to guarantee optimal service for every patient.

Qualifications:

Education:
High School Diploma or equivalent is required. Experience with EMR systems is preferred, along with a background in dermatology. A minimum of 2 years of experience in a medical office setting is necessary, with a strong emphasis on exceptional customer service.
Excellent written and verbal communication skills are essential, along with strong organizational abilities and attention to detail. Must be capable of managing multiple responsibilities and knowledgeable about OSHA and HIPAA regulations. Familiarity with medical and dermatological terminology is advantageous.

Physical Requirements:

This role involves significant time spent sitting, utilizing a phone and computer, with occasional lifting of 20-25 pounds. Candidates should be prepared for a physically active day, including walking, standing, and handling equipment, as well as lifting and moving items as needed.
