Job Overview

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at Optum Home & Community Care.

As a key member of our Senior Community Care (SCC) product, you will work closely with patients in their homes, nursing homes, and assisted living facilities to provide high-quality care and support.

About the Role

This is a full-time opportunity that offers a competitive salary range of $88,000 to $173,200 annually, depending on location and experience.

The successful candidate will have the opportunity to work as part of an interdisciplinary team to coordinate patient care, conduct comprehensive assessments, and develop personalized treatment plans.

Responsibilities

  • Conduct thorough medical evaluations and create detailed treatment plans to meet the unique needs of each patient.
  • Collaborate with patients, family members, and caregivers to ensure seamless communication and coordination of care.
  • Work closely with our care management team to identify and address potential health risks and develop strategies for prevention and intervention.
  • Participate in regular case reviews and quality improvement initiatives to continuously improve patient outcomes and enhance the overall quality of care.

Requirements

  • Master's degree in Nursing or a related field, along with active and unrestricted APC licensure in the state of Connecticut or ability to obtain by start date.
  • Active national NP or PA certification or the ability to obtain national certification in state of assignment by start date.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Access to reliable transportation.
