About the Opportunity:

We are seeking a highly skilled Clinical Care Coordinator for Children and Families to join our team at Child & Family Service. As a key member of our multidisciplinary team, you will be responsible for coordinating services, conducting assessments, and developing treatment plans that support healthy outcomes for children and families.

Key Responsibilities:

  • Conduct thorough assessments of clients and their families to identify areas for improvement.
  • Develop and implement personalized treatment plans that promote healthy coping mechanisms and parenting skills.
  • Collaborate with a multidisciplinary team to ensure seamless delivery of services.
  • Maintain accurate records and documentation as per Medicaid standards.
  • Make referrals for specialized evaluations and treatment when necessary.
  • Participate in treatment team meetings to ensure continuity of care.

Requirements and Qualifications:

  • Masters degree in social work or related field required.
  • Licenses and certifications (LHMC, LICSW, LMFT, etc.) as per Rhode Island Department of Health requirements.
  • A minimum of 6 months' experience working with children, adolescents, and/or families.
  • Strong organizational, critical thinking, and time-management skills.
  • Bilingual in Spanish preferred.

Compensation and Benefits:

We offer a competitive salary of approximately $62,000 - $72,000 per year, depending on experience, plus benefits including health insurance, retirement plan, and paid time off.
