Clinical Care Coordinator

The Masters-Level Clinician will be responsible for providing mental health services to individuals in crisis, including conducting assessments, developing treatment plans, and coordinating care with other healthcare providers.
Responsibilities:
- Conduct thorough assessments of clients' mental health needs and develop individualized treatment plans.
- Collaborate with clients, families, and healthcare providers to ensure comprehensive care and support.
- Develop and maintain relationships with community-based organizations and healthcare providers to promote effective care coordination.
- Participate in interdisciplinary team meetings to discuss client progress and develop strategies for addressing complex needs.
- Document client interactions and progress in accordance with established guidelines.
Requirements:
- LCDP or master's degree in counseling or related human services field required.
- LCSW, LICSW, or LMHC preferred.
- Previous experience in behavioral health and crisis level of care preferred.
- QMHP training required after entry for masters-level applicants.
Salary Range: $55,000 - $65,000 per year, depending on experience.
