About the Role

We are seeking a skilled Licensed Practical Nurse to join our Hospice team at Medi Home Hospice. The ideal candidate will possess a strong background in nursing and excellent patient care skills.

Key Responsibilities:

  • Deliver high-quality patient care, adhering to established plans of care and regulations.
  • Evaluate patient needs, developing and implementing personalized care plans as required.
  • Collaborate with interdisciplinary teams to ensure seamless patient care and communication.
  • Document patient progress, reporting any significant changes or concerns.

Requirements:

  • Graduate from an accredited LPN program.
  • Current licensure as a Licensed Practical Nurse in the state of NC.
  • At least one year of experience in direct patient care, preferably in home health or hospice settings.
