About the Job
We are seeking a highly skilled and empathetic Hospice Social Worker to join our team at our locations in St. Pauls (Robeson) NC, Raeford (Hoke) NC, Fayetteville (Cumberland) NC.
Job Responsibilities:
- Provide social work services in accordance with physician's plan of treatment.
- Work collaboratively with our interdisciplinary team to assess patient needs and develop effective support plans.
- Conduct assessments for normal and complicated grief, offering bereavement counseling as needed.
- Serve as a liaison between patients, families, and community resources.
Requirements:
- Masters or Bachelors degree in Social Work from a Council on Social Work Education accredited school.
- Current licensure to practice as a social worker in Virginia.
- Minimum one year hospice experience preferred.
- Valid driver's license.
