At (403500) Medi Home Hospice, we are seeking a skilled and compassionate Full-Time Hospice Social Worker to join our interdisciplinary team. This is an exciting opportunity to make a meaningful difference in the lives of patients and their families.

About the Role

We are looking for a highly motivated and organized individual with excellent communication skills to provide social work services in accordance with the physician's plan of treatment. As a Hospice Social Worker, you will work closely with the interdisciplinary team to assess patient needs, develop care plans, and provide emotional support to patients and their loved ones.

This role requires a Master's or Bachelor's level social work degree from a Council of Social Work Education accredited or approved school, as well as current licensure to practice as a social worker in the state of VA. A minimum of one year of Hospice experience is preferred, but not required. If you have a valid driver's license, are passionate about providing exceptional patient care, and thrive in a dynamic team environment, we encourage you to apply

  • Provide social work services in accordance with the physician's plan of treatment
  • Work with the interdisciplinary team to assess patient needs and develop care plans
  • Provide emotional support to patients and their loved ones
  • Serve as a liaison with other community resources
Requirements
