About the Role:We are seeking a highly motivated and compassionate individual to join our dynamic outreach team. As a Targeted Care Specialist, you will work closely with clients to help them apply skills learned in therapy in the community. This includes assisting individuals with primary diagnoses of Borderline Personality Disorder, PTSD, Major Depressive Disorder, and other mood disorders.
Key Responsibilities:- Work with clients to accomplish tasks such as social exposure, accessing community resources, applying for benefits, and securing housing.
- Follow through with medical treatment by successfully using skills such as interpersonal, DBT, anger management, problem-solving, and self-efficacy.
- Collaborate closely with clinicians on a multidisciplinary team, requiring critical thinking and strong organizational abilities.
- Complete progress notes on all clients seen and attend treatment team meetings and supervisions.
- Bachelor's degree in psychology, social work, rehab counseling education/counseling, or a related field from an accredited college or university.
- Valid driver's license and automobile with liability insurance in the amount required by agency policy.
- Fundamental computer experience to process email and client access.
- Experience with Microsoft Word and Excel is a plus.
