**Job Summary**

We are seeking a compassionate and experienced Registered Nurse to join our team as a Per Diem Hospice Nurse. As a member of our team, you will have the opportunity to work with a diverse range of patients and families, providing emotional support and care to those in need.

**Responsibilities**

  • Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Implement individualized plans of care and make revisions as needed.
  • Comply with AdvisaCare Hospice policies and procedures, as well as Medicare compliance requirements.
  • Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns.
  • Supervise paraprofessionals providing services to patients.
  • Prepare clinical and progress notes, and participate in interdisciplinary group conferences.
  • Participate in the hospice performance improvement program.
  • Maintain a high level of customer satisfaction with clients, families, and caregivers.
  • Communicate information using current processes and technology within the organization.

**Requirements**

  • Current Michigan license in good standing.
  • Minimum 1 year of hospice experience.
  • Empathetic approach to patients and caregivers.
  • Ability to adapt to patients' status and needs on a full-time on-call basis.
