Job Title: Registered Nurse - Hospice

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care, coordinate services, and support patients and their families during a challenging time.

Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Coordinate with healthcare providers, community resources, and other stakeholders to ensure seamless care delivery.
  • Communicate effectively with patients, families, and healthcare teams to ensure understanding and compliance with care plans.
  • Participate in interdisciplinary team meetings to discuss patient progress, address concerns, and develop strategies for improvement.
  • Document patient information, progress, and outcomes in a timely and accurate manner.
  • Collaborate with other healthcare professionals to ensure continuity of care and address any concerns or issues that may arise.

Requirements:

  • Graduate of an accredited nursing program with a current RN license in the state of employment.
  • At least one year of experience in hospice or palliative care, with a focus on case management and patient advocacy.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
  • Basic computer skills and proficiency in electronic medical records (EMRs).
