Job Summary

We are seeking a skilled Admissions Nurse to join our team at St. Croix Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and ensuring a seamless transition from hospital to home.

Responsibilities

  • Conduct comprehensive assessments of patients and families to determine hospice needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and performing various medical procedures.
  • Collaborate with the interdisciplinary team to coordinate patient care and ensure continuity of services.
  • Communicate effectively with patients, families, and healthcare professionals to ensure accurate and timely information exchange.
  • Participate in quality assessment and performance improvement initiatives to enhance patient outcomes and satisfaction.

Requirements

  • Graduate of an accredited school of nursing with current licensure in the state and CPR certification.
  • One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care.
  • Associate's degree required, bachelor's degree preferred.
  • One year of hospice care experience preferred.
