Peritoneal Dialysis Nurse Leader

To coordinate and oversee the care of patients in the assigned clinical area, ensuring seamless delivery of peritoneal dialysis services.
Key Responsibilities:- Assist the manager in making monthly staffing schedules and day-to-day work assignments to department dialysis staff.
- Assess workloads and make special assignments during peak and slow periods to ensure optimal patient care.
- Call in staff to meet minimum staffing requirements, notify managers of overtime requests, and coordinate with other clinical teams to cover staff shortages.
- Handle difficult and problematic work situations, including resolving member issues and escalating to management as needed.
- Serve as a resource for other staff members, answering questions, orienting new staff, and identifying training needs.
- Perform special projects assigned by the manager, such as implementing new workflows and processes, removing barriers, and communicating initiatives.
- Routinely provide feedback to the manager on staff performance, work habits, and barriers to efficient, high-quality work.
- Assist with the development and revision of department policies and procedures.
- Provide input into day-to-day operational issues that may support hiring, evaluation, and termination of staff decisions.
- Assess, plan, implement, and evaluate a plan of care for peritoneal dialysis patients, including administering medications, ensuring narcotics and drug supplies, and initiating and monitoring IV therapy and peritoneal dialysis exchanges.
- Provide ongoing assurance of functional emergency equipment and adequate emergency drug supplies.
- Document emergency situations within the department to the clinical operations manager.
- Set up, perform, and/or assist with procedures as appropriate.
- Ensure safety measures are instituted for all compromised patients receiving care.
- Coordinate, implement, and evaluate patient teaching plans for areas of diabetes mellitus, hypertension, anticoagulant therapy, therapeutic diet instruction, weight control, peritoneal dialysis at home, and other designated areas.
- Triages walk-in patients as needed or assigned.
- Supervises patient flow and directs other clinical support staff in the delivery of patient care within the Health Care Team (HCT).
- Acts as patient advocate and resolves problems within HCT.
- Minimum six (6) months experience providing peritoneal dialysis self-care training.
- Minimum one (1) year of current experience in a HMO or ambulatory setting.
- Minimum one (1) year experience in nursing care of the patient on home maintenance peritoneal dialysis or in nursing care of the end-stage renal patient.
- Bachelor's degree in nursing.
- Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire Registered Nurse License (District of Columbia) within 6 months of hire.
- Basic Life Support from American Heart Association.
- Must complete all Fundamental and assigned Specialty related competencies by the end of the probationary period and annually thereafter.
- Proficiency in the use of applicable computer software required.
- Experience in IV Therapy is required.
- Must have reliable transportation and a valid driver's license and able to complete a home assessment of patient's environment.
