Job Summary:

To oversee and coordinate the care of patients in the assigned clinical area, ensuring seamless delivery of peritoneal dialysis services.

Key Responsibilities:
  • Lead RN Duties - assist the manager with:
    • Creating monthly staffing schedules and day-to-day work assignments for dialysis staff.
    • Monitoring workloads and making special assignments during peak and slow periods.
    • Coordinating with other clinical teams to cover staff shortages and process provider schedule changes.
    • Handling difficult situations, including resolving member issues and escalating to management as needed.
    • Serving as a resource for staff members, answering questions, orienting new staff, and identifying training needs.
    • Performing special projects assigned by the manager, such as implementing new workflows and processes.
    • Providing feedback to the manager on staff performance, work habits, and barriers to efficient work.
    • Assisting with the development and revision of department policies and procedures.
    • Providing input on day-to-day operational issues that may support hiring, evaluation, and termination decisions.
  • Additional Regular RN Duties:
    • Assessing, planning, implementing, and evaluating care plans for peritoneal dialysis patients, including:
      • Administering medications and ensuring proper security of narcotics and controlled substances.
      • Initiating and monitoring IV therapy and peritoneal dialysis exchanges.
      • Providing ongoing assurance of functional emergency equipment and adequate emergency drug supplies.
      • Documenting emergency situations and ensuring safety measures are in place for compromised patients.
    • Coordinating and implementing patient teaching plans for areas such as diabetes mellitus, hypertension, and peritoneal dialysis at home.
    • Triage walk-in patients as needed or assigned.
    • Supervising patient flow and directing clinical support staff in delivering patient care.
    • Acting as a patient advocate and resolving issues within the Health Care Team.
Requirements:
  • Minimum six months experience providing peritoneal dialysis self-care training.
  • Minimum one year of current experience in an HMO or ambulatory setting.
  • Minimum one year experience in nursing care of patients on home maintenance peritoneal dialysis or end-stage renal patients.
Education and Credentials:
  • Bachelor's degree in nursing.
  • Registered Nurse License (Maryland) within six months of hire and Registered Nurse License (Virginia) within six months of hire.
  • Compact License: Registered Nurse within six months of hire.
  • Basic Life Support from American Heart Association.
Additional Requirements:
  • Must complete all Fundamental and assigned Specialty related competencies by the end of the probationary period and annually thereafter.
  • Proficiency in the use of applicable computer software required.
  • Experience in IV Therapy is required.
  • Must have reliable transportation and a valid driver's license and be able to complete a home assessment of patient's environment.
