HIV Case Manager Lead

We are seeking a highly skilled and compassionate HIV Case Manager Lead to join our team at San Ysidro Health Center. As a key member of our HIV Services department, you will be responsible for providing exceptional care and support to individuals living with HIV/AIDS.
Key Responsibilities- Assist clients in accessing HIV care services, including medical care, mental health counseling, and substance abuse treatment.
- Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of each client.
- Provide education and support to clients on HIV-related topics, including medication adherence, healthy living, and disease management.
- Collaborate with healthcare providers, social services agencies, and community organizations to ensure seamless care coordination.
- Maintain accurate and up-to-date client records, including progress notes, treatment plans, and outcome evaluations.
- Participate in quality improvement initiatives and contribute to the development of best practices in HIV case management.
- 3+ years of experience in human or social services, with a focus on HIV/AIDS care.
- Strong knowledge of HIV-related issues, including medical, psychosocial, and social service needs.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Bilingual (English/Spanish) required.
- Valid California driver's license and access to insured vehicle.
- A competitive hourly rate of $29.36 - $38.41.
