Behavioral Health Case Manager

The University of California, San Francisco (UCSF) is seeking a skilled Behavioral Health Case Manager to join our team at the UCSF Alliance Health Project (AHP). As a key member of our multidisciplinary team, you will provide case management services to individuals and groups who have Intravenous Drug Use (IVDU) and are at-risk of HIV and other comorbid diseases.
Key Responsibilities- Provide case management services to clients, including field work and home visits, to ensure engagement and linkage to substance use treatment and primary medical care services.
- Assist clients in creating and meeting treatment plan goals, reducing barriers to care, and providing supportive Harm Reduction-focused counseling services as needed.
- Work collaboratively with the Ward 86 HAP and POP-UP Teams to provide medical and preventive care services to at-risk clients.
- Develop and maintain relationships with community-based organizations and healthcare providers to ensure seamless care coordination.
- Participate in multidisciplinary team meetings to discuss client progress and develop treatment plans.
- Master's degree in Social Work, Psychology, Marriage and Family Therapy, or a related field from an accredited institution.
- Registration with the California Board of Behavioral Sciences (BBS) as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
- Experience in case management, substance use treatment, and Harm Reduction principles.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Experience working with HIV-positive and at-risk clients, LGBTQ+ communities, and intravenous drug users/substance users.
- Bilingual in English and Spanish, with written, reading, and oral proficiency.
- Familiarity with Avatar, Epic, ARIES, and Substance Use and Mental Health systems of care.
- Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities.
