Job Title: Licensed Practical Nurse

CFG Health Network is seeking a skilled Licensed Practical Nurse to join our team in a correctional healthcare setting. As a LPN, you will play a vital role in delivering high-quality patient care to inmates, working collaboratively with our healthcare team to ensure the best possible outcomes.

Responsibilities:
  • Perform and document medical evaluations on inmate patients, adhering to NCCHC, NJ 10-A, ACA, and PBNDS standards.
  • Conduct thorough assessments of inmate patients in various settings, including initial intake, healthcare units, and medical units.
  • Collaborate with primary care providers and other healthcare professionals to ensure continuity and timeliness of care.
  • Ensure accurate and complete medical record documentation, using SOAP format and problem-oriented approach.
  • Provide patient education and discharge planning, promoting a therapeutic environment and treating inmates with professionalism and respect.
Requirements:
  • Graduate from an accredited school of nursing program.
  • Current, unrestricted LPN license in the state where the facility is located.
  • At least one year of LPN experience, with preference for previous correctional healthcare experience.
  • Excellent critical thinking and assessment skills, ability to make decisions independently, and strong interpersonal communication skills.
