Join Our Team as a Licensed Practical Nurse

ConcertoCare is seeking a skilled Licensed Practical Nurse (LPN) to join our multidisciplinary Field Team, delivering home- and community-based care to patients with complex needs.

About the Role

As a Licensed Practical Nurse, you will be responsible for:

  • Partnering with team members to develop and implement clinical treatment plans under the supervision of providers and registered nurses.
  • Providing clinical services and coordination in the home- and field setting, including co-visits with other team members and independent LPN visits.
  • Conducting health screenings for newly enrolled patients and ongoing assessments.
  • Performing clinical services in the field, including wound care dressing changes, phlebotomy, medication review, and patient status checks.
  • Meeting with unable-to-reach patients in their home setting to re-engage them in the program.
  • Providing cross-coverage for planned and unplanned absences for team members to ensure patient care is not interrupted.
Requirements

To be successful in this role, you will need:

  • A licensed practice nurse, unencumbered license certification in the state of employment.
  • Experience providing care for the whole person, including patients living with mental illness and substance use disorders.
  • Current CPR and BLS certification.
  • Average to advanced computer & software skills (Microsoft Word, Excel, Outlook).
  • The ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation.
