Join Our Team as a Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at ConcertoCare. As a core member of our multi-disciplinary Field Team, you will deliver home- and community-based care to patients with complex needs.

Responsibilities

As a Licensed Practical Nurse, you will:

  • Partner with team members in the development and implementation of clinical treatment plans
  • Support wrap-around clinical services in the home- and field setting
  • Provide clinical services and clinical coordination services as directed by a patient's clinical risk assessment and clinical care plan
  • Conduct health screenings for newly enrolled patients and on an ongoing basis
  • Perform clinical services in the field, including wound care dressing changes, phlebotomy, medication review, and patient status checks
Requirements

To be successful in this role, you will need:

  • Licensed Practice Nurse, unencumbered license certification in the state of employment
  • Experience providing care for the whole person, including patients living with mental illness and substance use disorders
  • Current CPR and BLS certification
  • Average to advanced computer & software skills (Microsoft Word, Excel, Outlook)
