We are seeking a skilled Registered Nurse to join our team at Eden Health. As a Hospice Registered Nurse, you will provide in-home services to our patients throughout the Vancouver, WA metro area.
Key Responsibilities- Provide nursing services requiring substantial and specialized skill, in accordance with the plan of care.
- Manage a caseload of 12-18 patients, varying in complexity and geography.
- Develop and implement individualized care plans, addressing physical, emotional, psychosocial, and spiritual needs.
- Collaborate with interdisciplinary teams to ensure continuity of patient care.
- Educate patients and families on their rights and responsibilities as Hospice patients.
- Graduate of an accredited nursing program.
- Current licensed Registered Nurse (RN) in the state of practice.
- Experience as an RN in an acute-care setting, Home Health care, community health, or public health.
- Valid driver's license and reliable transportation.
- Proof of current CPR certification.
- Able to pass criminal background check and national sex offender clearance.
- Up-to-date COVID vaccine or exemption request.
- Flexible schedule to meet your needs.
