About the Role

We are seeking a highly skilled Nurse Practitioner to join our Senior Community Care team at UnitedHealth Group. As a key member of our interdisciplinary team, you will provide comprehensive care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities
  • Conduct thorough assessments and develop personalized care plans for patients with complex medical needs.
  • Manage acute medical situations and exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Collaborate with the care team to ensure seamless transitions and coordination of care.
  • Provide advanced illness and end-of-life discussions, as well as counseling and education to patients and caregivers.
Requirements
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date.
  • Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
Preferred Qualifications
  • 1+ years of clinical experience in a long-term care setting.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
