Join Our Team as a Home Health LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:
  • Provide professional nursing services under the direction of a RN in compliance with state laws and regulations, healthcare standards, and CenterWell Home Health policies.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans to promote optimal health outcomes.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress, identify areas for improvement, and make recommendations for enhanced care.
Requirements:
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
