Helio Health is seeking a highly skilled Senior Case Manager to join our team at our Syracuse Intensive Apartment Program. As a Senior Case Manager, you will play a vital role in the management and coordination of services to patients/residents participating in, or who have completed our treatment programs.
Key Responsibilities- Maintain a case load and perform primary case work responsibilities, including screening, intake, and orientation.
- Assess, evaluate, and intervene with patients/residents, as needed.
- Develop and implement treatment plans, as authorized.
- Refer patients/residents to community resources and services.
- Provide counseling and crisis intervention services.
- Conduct patient/resident education and training sessions.
- Coordinate care with other community providers and healthcare professionals.
- Develop and implement discharge plans.
- Train staff, volunteers, and students/interns on case management best practices.
- Participate in Quality Assurance activities and provide services according to agency policies, procedures, and governmental regulations.
- Associate's degree in a related field, Bachelor's degree preferred.
- Qualified Health Professional (QHP) certification preferred.
- Two years of experience working with behavioral health and/or substance abuse issues.
- Candidates with lived experience encouraged to apply.
- Valid NYS Driver's License preferred.
